Regarding insurance, Dr. Steven Masley is not enrolled as a “network health care provider” with any insurance carrier. If you have a PPO plan that allows you to go “outside” of their network of approved health care providers, a portion of your assessment may be eligible for reimbursement. We will give you a claim form after your appointment to submit. Any eligible reimbursement will go directly to you.

INSURANCE REIMBURSEMENT POLICY:

Dr. Masley’s Optimal Health Center is not enrolled as an “in-network healthcare provider” in any medical plans as most medical insurance plans anticipate a 20 to 30-minute physical and/or a 12-minute office visit; they reimburse accordingly. As we spend many hours working to optimize your health, we have declined to enroll as an “in-network provider” which would have forced us to minimize our time together.

For your visit, payment is due on the day of your appointment unless other arrangements have been agreed upon in advance. We will provide you with an insurance claim form for you to mail and seek reimbursement for your medical expenses incurred.

We have worked with a consultant to maximize the reimbursement you are legally permitted.

If you have a PPO plan or an insurance plan that allows you to go “outside” of their network of enrolled healthcare providers, a portion of your assessment should be eligible for reimbursement. We provide a HCFA claim form and any reimbursement will go directly to you. Ask your insurance carrier if you have the option to go “outside of network”. Clarify as well whether you can select a physician yourself, or if the out-of-network visit requires a referral from an “in-network provider”.

If you have an HMO plan or a plan that requires you to be seen “in-network” it is very unlikely that your assessment with us would be covered by your insurance plan. Of course you are encouraged to schedule an appointment with use to optimize your health in ways that a medical insurance plan will not.

Medical insurance is intended to cover emergency health problems, and to treat (not reverse) medical problems. If you want to optimize your health and prevent problems before they occur, likely you need to cover some of this expense. Please remember, optimal health is your single most important asset; don’t neglect it.

CANCELLATION POLICY:

Unless a corporate arrangement has been agreed upon in advance, a $500 deposit is required at the time you confirm your appointment date. (As we only schedule 1-3 evaluations per day and are usually unable to schedule patients in less than 2 weeks, cancelling your appointment results in a lost evaluation for our center.) Cancelling your appointment with less than 14 days notice could result in losing the down payment. You may of course send another person in your place for an OHC comprehensive evaluation, or a 40-50 Plus Evaluation, without any risk of losing your deposit; however, the substituting person will need to complete their paperwork by the day of their visit.
